Reservation Request Form
In-Person Attendance Requirements:
  • Reservations are required for all attendees - including children of all ages.  Please fill out the Reservation Request Form by Wednesday afternoon each week that you would like to attend.  You will receive email notification of whether you will be able to attend services by Thursday afternoon.
    • Seating in the Main Sanctuary is limited to 35 people.
      • Seating priority will be given to members of Emanuel Synagogue who have a yahrzeit, anniversary, or milestone birthday.
      • Second priority will be given to members of Emanuel Synagogue.
      • Third priority will be given to members of Rabbi Jacobson's conversion class, and to family of Synagogue members.
  • Attendees must comply with all written and verbal instructions  - from staff, security personnel, and synagogue volunteers - while in the building.
  • The doors will be open from 9:45 am to 10:15 am.  Those arriving earlier must wait in the parking lot.  Those arriving later than 10:15 am will not be admitted.
  • All attendees (age 2 and older) must wear a mask, covering nose and mouth, at all times while in the building.  This includes attendees who have been fully vaccinated.
  • The temperature of any attendee may be taken at the door - those running a fever may not enter the building.
  • Covid-19 screening questions may be asked of anyone entering the building.  The Synagogue reserves the right to deny entry to the building on the basis of answers to these questions:​
​​                    -Do you have a recent onset of any of the following potential Covid-19 related symptoms:  cough, shortness of breath, fever (100.4F or greater), sore throat, muscle aches, loss of smell or taste, or gastrointestinal issues?
​​                    -Have you had a positive Covid-19 infection test, taken from the nose or throat, within the past two weeks?
​​                    -Have any of your family members or other with whom you have been in close contact had positive tests for Covid-19 infection, taken from the nose or throat, within the past two weeks?
​​                    -Have you been advised to self-isolate by a healthcare provider?
​​                    -In the past two weeks, have you been physically present with ten or more people where social distancing was not practiced and masks were not worn?
